OGBGA ALL-STATE NOMINATION FORM
Guidelines:
1. Head coach must be a member of the Oklahoma Girls Basketball Coaches Association in order for the player to
receive the honor. Attach a photo copy OR your coach’s card (or receipt) with this application.
2. Must have ONE(1) wallet-size pictures. A player cannot be considered for selection unless the picture is with this application.
3. Must have a list of honors on a separate sheet of paper and attached to this form.
4. Must attend workouts unless excused by Executive Group.
5. If a girl is selected, she will need to let her All-State Coach know that she is playing by May 1st or be replaced! If no communication is received, the player will automatically be replaced on May 2nd.
6. Please mail this form along with: picture (1), attached copy of your coach’s card (or receipt), to your District Rep. or handed to him/her at the first tryout. Please also include the list of honors
and activities on a separate sheet of paper and attach it to this form.
